| You can bring the forms to the Kick off night Sept. 16, to Palmer Soderberg at 3730 40th Ave NW or mail it in with your payment to the address provided on the forms. Registration is due October 15th, 2008. ($25.00 Late fee after October 15th) |

The remaining forms can be turned in with your registration forms or by the first practice of November 10, 2008. |

Physicals are required and are due by the first practice November 10th, 2008. |

If you have an up to date physical on file with your schools Athletic Department you can request a copy of it and send that in with your forms. |
